Adenoid cystic carcinomas (ADCC) are distinctive salivary gland neoplasms with characteristic histomorphology. The diagnosis of dedifferentiation/high-grade transformation (HGT) indicates poor prognosis and is most often made on histopathology. We present a case of ADCC arising from a minor salivary gland tumor exhibiting HGT, reaching up to the submandibular gland and having lymph node metastases, suspected on fine-needle aspiration cytology.

Adenoid cystic carcinoma (ACC) is a major histological type of salivary gland cancer but an uncommon form of vulvar cancer. Salivary gland ACC occasionally dedifferentiates into high-grade carcinoma, resulting in poor prognoses. The dedifferentiated component is usually a poorly differentiated cribriform or solid carcinoma, whereas squamous cell carcinoma (SCC) is exceptional.

Article Synopsis
  • This study examines high-grade transformation (HGT) in adenoid cystic carcinoma (ACC), revealing its aggressive behavior and the serious clinical consequences for patients.
  • Researchers compared 23 HGT patients to non-HGT patients, finding HGT primarily in sinonasal and lacrimal glands, with a worrying increase in lymph node metastasis, perineural invasion, and tumor growth.
  • The findings indicate that patients with HGT face higher rates of recurrence and distant metastasis, suggesting they should be classified as Grade 4 due to their significantly worse survival rates compared to non-HGT cases.

Lacrimal gland adenoid cystic carcinoma (AdCC) is associated with an aggressive clinical course and grave prognosis. A high grade transformation within adenoid cystic carcinoma of lacrimal gland is a rare condition which is even more locally aggressive with frequent neck and distant metastasis. We present a case of left lacrimal gland adenoid cystic carcinoma with high grade transformation to adenocarcinoma NOS type presenting with orbital pain and proptosis.

Article Synopsis
  • High-grade transformation (HGT) and dedifferentiation occur in various salivary gland carcinomas, including acinic cell and adenoid cystic carcinoma, leading to a mix of low-grade and high-grade tumor features.
  • The high-grade component is characterized by poorly differentiated cells with distinct microscopic changes, higher cell proliferation rates, and aggressive behavior, resulting in poorer patient outcomes.
  • The exact molecular mechanisms behind HGT are not fully understood, but changes like p53 inactivation and increased HER2 levels have been observed; hence, thorough examinations of salivary gland tumors are critical to identify high-grade areas for better treatment planning.
