Introduction: Despite continued improvement in post-sepsis survival, long term morbidity and mortality remain high. Chronic critical illness (CCI), defined as persistent inflammation and organ injury requiring prolonged intensive care, is a harbinger of poor long-term outcomes in sepsis survivors. Current dogma states that sepsis survivors are immunosuppressed, particularly in CCI.

Background: Delays and missed opportunities for timely treatment contribute significantly to stark inequities in cervical cancer mortality in low- and middle-income countries (LMICs) compared to high-income countries. The vast majority (approximately 90%) of new cases and deaths occur in LMICs, particularly those with high rates of HIV such as Botswana. To date, most of the implementation and cancer control research in Botswana and other LMICs has focused on cancer prevention and screening, with limited focus on cancer treatment.

Background: NF1 inactivation is associated with sensitivity to MEK inhibitor targeted therapy in low-grade and some high-grade gliomas. NF1 loss may also be a harbinger of exploitable vulnerabilities in IDH-wildtype glioblastoma (GBM). Accurate and consistent detection of NF1 loss, however, is fraught given the large gene size, challenges with complete coverage and variant calling upon sequencing, and mechanisms of mRNA and protein regulation that result in early degradation in the absence of genomic alterations.

Brief Resolved Unexplained Event: Evidence-Based and Family-Centered Management.

Pediatr Rev

October 2024

Division of Hospital Medicine, Department of Pediatrics, Seattle Children's Hospital, University of Washington, and Seattle Children's Research Institute, Center for Clinical and Translational Research, Seattle, WA.

Article Synopsis
  • Brief resolved unexplained events (BRUEs) are common in infants under 1 year, characterized by being asymptomatic with no clear diagnosis; less than 5% are linked to serious conditions.
  • Despite the low risk of serious issues, about 63% of BRUE cases lead to hospitalization, and up to 82% result in diagnostic testing, often leading to false positives.
  • Effective management of BRUE requires balancing reassurance for anxious parents while explaining risks versus benefits of tests, with a family-centered approach to decision-making that respects caregivers' values.
