Extra-articular symptoms, including headaches, are frequently encountered in patients with Ehlers-Danlos syndrome (EDS) and hypermobility spectrum disorders (HSD), and may be the presenting complaint. Migraine is reported in up to three quarters of patients with symptomatic joint hypermobility, have a higher headache frequency, and an earlier age of onset compared to the general population. Orthostatic headache is an important presentation, and should raise suspicion of an underlying spinal cerebrospinal fluid leak, dysautonomia, and craniocervical pathology, which are all associated with heritable connective tissue disorders (HCTD) including EDS.

The management of persisting instability following anterior cruciate ligament reconstruction (ACLR) includes isolated lateral extra-articular tenodesis (LEAT). The present study investigates the outcomes following isolated LEAT to address ongoing instability following ACLR without LEAT. Patients with ongoing symptomatic instability following ACLR with a radiologically intact and well-positioned graft who underwent an isolated LEAT between January 2017 and March 2022 were retrospectively recruited.

Pleural effusions and pulmonary nodules are known complications of rheumatoid arthritis (RA) though pleural extra-articular manifestations without clinical arthritis are rare. We present a 66-year-old male with dyspnea and weight loss, whose imaging revealed pleural effusions and lung nodules, multiple exudative pleural effusions, and a thoracoscopic biopsy showing chronic pleuritis and granulomatous inflammation. Subsequent joint stiffness and positive rheumatoid markers led to a RA diagnosis.
