Large-scale diversion of controlled substances (CS) from within a hospital or heath system pharmacy is a rare but growing problem. It is the responsibility of pharmacy leadership to scrutinize control processes to expose weaknesses. This article reviews examples of large-scale diversion incidents and diversion techniques and provides practical strategies to stimulate enhanced CS security within the pharmacy staff. Large-scale diversion from within a pharmacy department can be averted by a pharmacist-in-charge who is informed and proactive in taking effective countermeasures.

Background: Intrasaccular flow diversion using the woven endobridge device (WEB; MicroVention, Aliso Viejo, CA, USA) for the treatment of intracranial aneurysms has demonstrated large scale safety and efficacy. However, limitations arise from its structural configuration, restricting its application to specific aneurysm sizes and shapes.
Technique Overview: We introduce the CUPCAKE technique, a combination of conventional coiling followed by WEB intrasaccular flow disruption in select cases of atypical aneurysms with technically challenging morphology not typically treatable by WEB alone.

Long-distance water diversion projects typically utilize various hydro-engineering facilities, creating complex and dynamic habitats. However, the microbial dynamics of multi-trophic microorganisms during water diversion and their responses to different hydro-engineering habitats remain poorly understood. California's organic waste diversion law, SB 1383, mandates a 75% reduction in organics disposal by 2025 to reduce landfill methane emissions. Composting will likely be the primary alternative to landfilling, and 75-100 new large-scale composting facilities must be sited in the state to meet its diversion goal. We developed a strategy for evaluating site suitability for commercial composting by incorporating land-use, economic, and environmental justice criteria.
