Chordal replacement has been a tool in mitral repair for two decades and is a particularly suitable option for the treatment of anterior mitral leaflet (AML) prolapse. Chordal replacement has also become an alternative to more conventional techniques in posterior leaflet prolapse. It has proven to be safe and durable, and the mid- to long-term results in AML repair are approaching those achieved by conventional repair of the posterior leaflet.

To evaluate the clinical outcomes of combined complete preservation of chordal structure mitral valve replacement (C-MVR) with total anatomical arterial myocardial revascularization (TACR) in coronary patients with moderate-to-severe or severe ischemic mitral regurgitation (IMR). Background: Mechanisms of repair failure after mitral valve repair (MVr) using chordal replacement and annuloplasty for degenerative mitral regurgitation were analyzed.
Methods: All mitral valve reoperations after isolated MVr using solely chordal replacement and annuloplasty for degenerative mitral regurgitation at the German Heart Center Munich (Munich, Germany) were reviewed. This retrospective observational study aimed to analyze mechanisms of repair failure leading to reoperations.

The use of the leadless pacemaker system (Micra; Medtronic) has grown in popularity with a lower risk of postimplantation complications associated with traditional pacemakers. We describe the case of a 78-year-old woman who presented with torrential tricuspid regurgitation due to avulsion of the anterior papillary muscle and entire chordal apparatus of the tricuspid valve after Micra pacemaker placement at an outside hospital. After she was deemed ineligible for transcatheter approaches, she underwent a minimally invasive tricuspid valve operation.
