Surgery for large tumors that occupy one side of the thoracic cavity requires a surgical strategy that considers not only the localization and size of the tumor but also tumor manipulation, access to adjacent vital organs such as the pulmonary arteriovenous, the tumor infiltration range, and combined organ resection. To perform safe and high-quality surgery, it is necessary to obtain a good surgical field, and determining the optimal surgical approach to use is also important. Thoracotomies include posterolateral thoracotomy, rib-cross thoracotomy, clamshell thoracotomy, hemi-clamshell thoracotomy, and transmanubrial osteomascular-sparing approach.

A 37-year-old male, with a 5-year history of liposarcoma of the right thigh, was incidentally diagnosed with two huge thoracic metastases following a fall. One of these masses, measuring 22 cm, was located in the right chest apex, adjacent to a second 20 cm mass situated in the anterior mediastinum, partially invading the left chest. The patient underwent surgical intervention for mass resection that commenced with a hemi-clamshell incision, but was then extended by completing the lower median sternotomy in order to create a T shaped incision.

Solitary fibrous tumors (SFTs) are rare mesenchymal pleural neoplasms with an overall good prognosis and low recurrence rate if completely resected and if degree of differentiation is favorable. Within the last decade, advances in research have led to more reliable methods of differentiating SFTs from other soft tissue tumors. Historically, several markers were used to distinguish SFTs from similar tumors, but these markers had poor specificity.
