In a group of 29 patients with mechanical icterus and a control group of 38 patients (acute, chronic hepatitis and hyperbilirubinemia) there have been followed up alkaline phosphatase, leucinaminopeptidase and gammaglutamil transpeptidase activities aiming to evaluate the value of gamma GT in differentiation of icterus of various etiology. Comparing the values of the examined cholestase enzymes in patients of obstructive icterus it could be seen that gamma GT was positive in the largest percentage of the diseased, (69,6%), then APH (89,7%) AND LAP (86,3%). Gamma GT has shown to be selective test for obstruction of hepatobiliary tract but it is not selective for discovering cause of obstruction. It can be explained by the fact that activity of gamma GT could be found both in mechanical icterus caused by calculosis or tumours (with a very high activity) and in patients with hepatitis with cholestatic component, although in the decresed activity.

End-stage liver disease includes liver failure and decompensated cirrhosis resulting from various etiologies and often leads to patient mortality due to complications and clinical symptoms such as severe jaundice, ascites, hepatic encephalopathy, coagulopathy, and hepatorenal syndrome. Liver transplantation is currently regarded as the most effective treatment, but its clinical application is limited by the shortage of donors, elevated expenses, and post-transplant rejection. Stem cells are a group of cells with multidirectional differentiation potential and self-renewal ability, which can improve the clinical indicator outcomes through mechanisms such as immunoregulation and promotion of tissue repair in patients with end-stage liver disease.

We report the case of a 24-year-old man from Brazil presenting with jaundice and epigastric pain. Abdominal CT and endoscopic ultrasound (EUS) revealed a mass in the pancreatic-duodenal groove, intrahepatic duct dilation, and lymphadenopathy, initially suggestive of lymphoproliferative syndrome. However, cytopathological analysis of EUS-guided fine needle aspiration (EUS-FNA) of the lymph nodes confirmed paracoccidioidomycosis.

Peribronchiolar metaplasia is an uncommon lesion characterized by fibrosis and bronchiolar epithelial cell proliferation along the peribronchiolar alveolar walls, primarily in response to bronchiolar and peribronchiolar injuries. Peribronchiolar metaplasia usually appears as ground glass nodules or sub-solid nodules on computed tomography. However, we present an exceptional case of peribronchiolar metaplasia that appeared as a solitary solid nodule on computed tomography.
