We present an unusual case of syphilis that despite starting treatment with penicillin has evolved with renal complications. Due to glomerulonephritis with nephrotic syndrome the case required shared care between the renal physician and dermatologist. Doctors of different specialties need to keep in mind that early syphilis can have visceral involvement and patients might need prompt, specific therapeutic intervention.

National case-based surveillance data show that reported neurologic manifestations of syphilis increased during 2019-2022 among persons with early and late-stage syphilis. Neurologic manifestations occurred across demographic groups and among those with and without HIV, highlighting the importance of evaluating for neurologic signs and symptoms in all persons with syphilis.

The incidence of sexually transmitted infections (STI) is also prevalent in the cases of sexual abuse in this population, showing the need for comprehensive medical and psychological intervention. This study presents a case of a 15-year-old Indonesian MSM adolescent who experienced three forced sexual intercourse with a mid-thirties male friend 6 months before the consultation.
