Unlabelled: This case is a rare presentation of breast implant-associated anaplastic large cell lymphoma (BIA-ALCL), where malignant pericardial effusion (MPE) served as the primary manifestation. A 58-year-old woman, post-breast implant removal, presented with pleuritic chest pain, fever, and chills. Clinical evaluation revealed jugular venous distention, muffled heart sounds, and hemodynamic instability.

Medical devices used for functional or esthetic purposes improve health and quality of life; however, they are not risk-free. Anaplastic large-cell lymphoma (ALCL), associated with breast implants, is a well-known and recognized distinct lymphoma entity. More recently, additional lymphomas have been reported in relation to prosthesis other than breast implants, as these allow the pericyte to develop into a clone that undergoes a maturation process, progressing toward full malignancy.

The voluntary recall of textured breast implants due to their association with breast implant-associated anaplastic large cell lymphoma has resulted in the loss of the primary advantage of the textured surface: positional stability. We have engineered a novel soft gel-filled smooth implant with a surface that promotes positional stability without texture, known as the positionally stable smooth implant (PSSI). Miniature anatomically shaped breast implant shells were fabricated from polydimethylsiloxane using 3D-printed molds.

Continuous research on breast implant-associated anaplastic large cell lymphoma (BIA-ALCL) has introduced a focus on surface texturizations and a shift towards smooth breast devices, yet outcomes comparing the complication profiles of differently textured tissue expanders (TEs) remain conflicting. The study aim was to compare the complication profile of a new nanotextured and MRI-compatible TE to micro- and macrotextured TEs and to identify possible predictors for complications. A retrospective analysis of women undergoing expander-based breast reconstruction after mastectomy between January 2016 and March 2022 was conducted.
