We present a rare case of probable caseous calcification of the mitral. This pathology is more frequently detected in asymptomatic women older than 70 years. To recognize this image is important because echocardiography is the easiest way to elucidate this diagnosis, and more importantly because this structure could be easily misdiagnosed as tumors, thrombus and vegetations, which are much more common. Normally, it has a benign evolution, and the correct diagnosis is crucial to avoid unnecessary surgical interventions.

BACKGROUND Echogenic masses in the mitral annulus are often incidental findings during routine echocardiography examinations. Most represent the wide spectrum of underlying mitral annular calcifications and usually do not require further diagnostic workup. However, a rare variant called "caseous calcification of the mitral annulus" (CCMA) can be challenging to diagnose and often requires an extensive workup with the use of multiple imaging studies in order to accurately confirm the pathology.

Materials: The natural mummy of an anonymous friar from the mortuary chapel of the church of Santa Maria della Grazia in Comiso (Sicily) METHODS: The mummy was studied through macroscopic examination; tissue sampling was conducted through breaches in the dorsal surface of the thorax. Radiological, histological and immunohistochemical analyses were performed on the pulmonary parenchyma.
