Extracorporeal membrane oxygenation (ECMO) has emerged as a salvage therapy in refractory respiratory failure. Within the literature, there is evidence to support the use of ECMO in severe thoracic trauma; however, there is minimal information on its applicability in mild to moderate thoracic trauma. This report describes a man in his 50s who suffered thoracic trauma following a motor vehicle accident, who, despite maximal medical therapy, experienced deterioration in respiratory function, requiring the commencement of veno-venous ECMO on day 5 post injury and subsequently achieved excellent functional recovery.

The superiority of veno-arterial over veno-venous extracorporeal membrane oxygenation for operative support of lung transplantation.

BMC Pulm Med

January 2025

Department of Critical Care Medicine, Sichuan Provincial People's Hospital, Sichuan Academy of Medical Sciences, University of Electronic Science and Technology of China, Chengdu, 610072, China.

Background: Veno-arterial (V-A) and veno-venous (V-V) extracorporeal membrane oxygenation (ECMO) are crucial support modalities during lung transplantation, yet their comparative effectiveness remains unclear.

Methods: We conducted an 8-year retrospective analysis of 62 lung transplant recipients who received intraoperative ECMO (29 V-A, 33 V-V). Baseline characteristics, surgical parameters, and clinical outcomes were compared.

Introduction: Acute kidney injury (AKI) is a common complication of acute respiratory distress syndrome (ARDS) and multiple organ dysfunction syndrome (MODS) in patients receiving extracorporeal membrane oxygenation (ECMO) support, leading to requirement of continuous renal replacement therapy (CRRT) in 70% of ECMO patients. Parallel arrangement of CRRT and ECMO circuits is common in adult patients. However, CRRT may also be integrated directly into the ECMO circuit.

Severe accidental hypothermia can lead to cardiac arrest. The most efficient method of resuscitating and warming is by ECMO (Extracorporeal Membrane Oxygenation). While the convention is to use VA ECMO (Veno Arterial ECMO), using VV ECMO (Veno Venous ECMO) in which the blood is returned directly into the right ventricle could be an alternative and lead to conversion to life sustaining cardiac rhythm.

Multi-disciplinary treatment of broncho-esophageal fistula in a high-risk single-lung patient.

J Cardiothorac Surg

January 2025

Section of Cardiothoracic Surgery, Department of Heart Disease, Haukeland University Hospital, Jonas Lies vei 65, 5021, Bergen, Norway.

Background: A broncho-esophageal fistula (BEF) is a medical and surgical disaster. Treatment of BEF is often limited to palliative stent treatment that may migrate or cause erosions and tissue necrosis. Surgical repair of BEF is the only established definite treatment.
