Although it is not uncommon for patients with Chiari I malformations to present with respiratory complaints, cough syncope is a rare presenting symptom. We report an adult patient who had both a Chiari I malformation and atlanto-occipital assimilation, and complained of cough syncope, orthopnea, and central sleep apnea. The patient underwent decompressive craniectomy of the posterior fossa and a cervical level 2 laminectomy. However, due to an initial under-appreciation of the profound narrowing of the foramen magnum as a result of these concomitant pathologies, the patient had continued impaired cerebrospinal fluid flow, leading to a symptomatic pseudomeningocele and required a more extensive decompression that included a cervical level 3 laminectomy as well as a temporary lumbar drain. On 2 year follow-up, he remained asymptomatic.

Cardioneuroablation is a rapidly developing procedure for the treatment of vagally mediated bradyarrhythmias. However, the lack of multicenter, randomized trials prevents it from being included in bradyarrhythmia treatment guidelines. So far, only one small, randomized study has been published assessing the effectiveness of this method in the treatment of reflex syncope.

Background: An association between testicular cancer and Down syndrome has been reported by several studies. Down syndrome with cryptorchidism and retroperitoneal mixed germ cell tumours is rare, and yolk sac tumours are often considered secondary components of mixed germ cell tumours. Herein, we present a rare case of retroperitoneal mixed germ cell tumour with cryptorchidism accompanied by yolk sac tumour and seminoma in a patient with Down syndrome, along with its imaging features.

When undergoing or about to undergo a needle-related procedure, most people are not aware of the adverse emotional and physical reactions (so-called vasovagal reactions; VVR), that might occur. Thus, rather than relying on self-report measurements, we investigate whether we can predict VVR levels from the video sequence containing facial information measured during the blood donation. We filmed 287 blood donors throughout the blood donation procedure where we obtained 1945 videos for data analysis.

Introduction: A leadless pacemaker (LLPM) was recommended for a patient with intermittent complete heart block and near-syncope.
Methods And Results: Delivery of LLPM is through a large sheath that has limited deflection and steerability. This report describes the successful deployment of a ventricular LLPM in a patient with prior surgical correction of AV septal defect with subsequent significant right atrial enlargement.
