Article Synopsis
  • The purpose of this project was to update the 2016 recommendations regarding the management of immunosuppressants or biologics in patients with giant cell arteritis (GCA).
  • A task force of 18 physicians developed 26 validated recommendations after thorough discussion and a >85% consensus process.
  • Key recommendations include using subcutaneous tocilizumab (TCZ) as a first-line treatment when glucocorticoid-sparing is needed, initiating TCZ at diagnosis for patients with specific high-risk conditions, and considering TCZ or methotrexate if glucocorticoid discontinuation is not feasible after 12 months.

Multidisciplinary approaches in the management of advanced hepatocellular carcinoma: Exploring future directions.

World J Gastrointest Oncol

October 2024

Department of Gastroenterology and Hepatology, West China Hospital, Sichuan University, Chengdu 610041, Sichuan Province, China.

Recently, we read the article "Pathologically successful conversion hepatectomy for advanced giant hepatocellular carcinoma after multidisciplinary therapy: A case report and review of the literature" published in the . The prognosis of advanced hepatocellular carcinoma (HCC) is poor, and multidisciplinary comprehensive treatment is currently the main research direction. This case report demonstrated the efficacy of the combination therapy of transcatheter arterial chemoembolization, hepatic arterial infusion chemotherapy, epclusa, lenvatinib and sintilimab for a patient with advanced HCC, and the report can serve as a reference for clinical practice.

Deep Learning to Discriminate Arteritic From Nonarteritic Ischemic Optic Neuropathy on Color Images.

JAMA Ophthalmol

November 2024

Department of Neuro-Ophthalmology and Rothschild Neurovisual and Computational Sciences BRAIN Lab, Rothschild Foundation Hospital, Paris, France.

Article Synopsis
  • The study focuses on improving the diagnosis of arteritic anterior ischemic optic neuropathy (AAION) from giant cell arteritis, which can prevent vision loss, particularly when symptoms are subtle or unreliable tests are involved.
  • Researchers developed a deep learning system (DLS) to differentiate between AAION and nonarteritic anterior ischemic optic neuropathy (NAION) by analyzing color fundus images from a large international dataset involving 961 eyes from 802 patients.
  • The DLS demonstrated promising performance metrics, such as area under the receiver operating characteristic curve (AUC), sensitivity, specificity, and accuracy, validating its potential as a diagnostic tool during the acute phase.
