Background: Although the association of short-term ozone and heatwave exposure with cerebrovascular disease has been well documented, it remains largely unknown whether their co-exposure could synergistically trigger ischemic stroke (IS) mortality.

Prehospital management of exertional heat stroke (EHS) consists of monitoring rectal temperature (T) while aggressively cooling via cold water immersion. Recent recommendations suggest using central nervous system (CNS) dysfunction to determine cessation of cooling when T is not available. We examined cognitive responses of two runners with EHS after a road race.

Heat stroke is characterised by hyperthermia and acute encephalopathy. We describe a rare case of classical heat stroke secondary to prolonged sauna use with multiorgan sequelae including seizures, liver injury, kidney injury, disseminated intravascular coagulation, rhabdomyolysis and type 2 myocardial infarction. The patient was treated with external cooling, intravenous fluid therapy and blood products, and made a full recovery without need for advanced organ support.

This case report describes a patient with a medical history of schizophrenia, found in a coma with hyperthermia, likely due to classic heatstroke. The white blood cells observed on the blood smear showed cytological abnormalities characterized by multilobed nuclei, which could be early signs of cell death. The evolution into multiorgan failure led rapidly to death.

Heat shock proteins (HSPs), essential for maintaining protein stability and cellular homeostasis, are pivotal in neuroperotection. Consequently, deepening the understanding the role of HSPs in ischemic stroke (IS) risk is crucial for identifying novel therapeutic targets and advancing neuroprotective strategies.
