Background: Preventing hepatic encephalopathy (HE) recurrence in cirrhosis, which is associated with an altered gut-liver-brain axis, is an unmet need. Fecal microbiota transplantation (FMT) is beneficial in phase-1 studies, but route and dose-related questions remain.
Methods: We performed a phase-2 randomized, placebo-controlled, double-blind, clinical trial of capsule and enema FMT in cirrhosis and HE on lactulose and rifaximin.

Hepatic de novo lipogenesis (DNL) is increased by both carbohydrate intake and protein consumption. In hepatic fat synthesis, a key role is played by the induction of the hepatic expression of lipogenic genes, including Fasn, Scd1, and Srebf1. Regarding carbohydrate intake, increased blood glucose and insulin levels promote the expression of hepatic lipogenic genes.

This study aimed to evaluate the efficacy of vinegar-soaked Ficus carica fruits (VSFCF) for managing cholelithiasis in adults. A parallel-group, single-blinded, randomized controlled trial at Noreen Nishat Welfare Hospital, Khanewal, Pakistan, was conducted from December 2023 to February 2024. Fifty adults with ultrasound-confirmed cholelithiasis were randomly assigned to receive either VSFCF (1 g/kg/day) or ursodiol (600 mg/day).
