Cardiovascular status in endogenous cortisol excess: the prospective CV-CORT-EX study.

Eur J Endocrinol

November 2024

Department of Internal Medicine I, Division of Endocrinology and Diabetes, University Hospital, University of Würzburg, 97080 Würzburg, Germany.

Objective: Cushing's syndrome (CS) results in increased cardiovascular (CV) morbidity and mortality. Subtype-specific differences and possible reversibility after biochemical cure are not well investigated.

Design: Prospective cohort study evaluating the CV status in different forms of endogenous cortisol excess.

Incidentalomas have an increasing incidence all over the world due to a larger access to imaging assessments, and endocrine incidentalomas make no exception in this matter, including pituitary incidentalomas (PIs). Our objective was to analyse the dynamic changes amid a second computed tomography (CT) scan after adult patients were initially confirmed with a PI (non-functioning micro-adenoma). This was a multi-centric, longitudinal, retrospective study in adults (aged between 20 and 70 y) amid real-world data collection.

Who and how to screen for endogenous hypercortisolism in adrenal and pituitary incidentaloma.

J Endocrinol Invest

October 2024

Section of Endocrinology, Geriatrics and Internal Medicine, Department of Medical Sciences, University of Ferrara, Ferrara, Italy.

Article Synopsis
  • Adrenal incidentalomas (AIs) and pituitary incidentalomas (PIs) are common findings due to increased imaging, with this review focusing on the prevalence of hypercortisolism in these patients.
  • The study used PubMed to analyze demographic data and diagnostic accuracy regarding hypercortisolism and found significant links between mild autonomous cortisol secretion (MACS), cardiometabolic risks, and higher mortality.
  • Early detection and management of hypercortisolism are crucial to prevent complications, highlighting the need for targeted screening in high-risk individuals.

Context: The clinical presentation of Cushing's syndrome (CS) overlaps with common conditions. Recommended screening tests are serum cortisol after 1-mg overnight dexamethasone suppression test (DST), urinary free cortisol (UFC), and late-night salivary cortisol (LNSC).

Methods: We analyzed the diagnostic accuracy of screening tests in 615 patients without CS (263 suspected CS, 319 adrenal and 33 pituitary incidentaloma) and 40 with CS.
