Introduction: Ileal conduit (IC) is the standard urinary diversion following radical cystectomy. The formation of stone in the IC is one of the relatively common late complications of the procedure.

Case Report: The case of 69-year-old man who developed 10-cm large stone in the IC, six years after cystectomy is presented.

Conclusion: The majority of patients with the stone in IC can be treated with minimally invasive techniques, like manual extraction, or endoscopic procedures.
