Endovascular treatment of aortic arch aneurysms with a proximal landing zone in zone I requires carotid-carotid bypass. We report two patients with aortic arch aneurysms in whom we used a new hybrid approach by transposing the left common carotid artery and left subclavian artery to the right common carotid artery. This innovative technique uses a native vessel instead of prosthetic material and a shunt to maintain cerebral perfusion, thereby avoiding bilateral carotid clamping.

Methods: We utilized the GSE213740 single-cell sequencing dataset to assess PANoptosis levels in VSMCs. Datasets GSE153434 and GSE147026 were employed to identify differentially expressed genes (DEGs) and perform weighted gene co-expression network analysis.

Purpose: To demonstrate the feasibility of the physician-modified endovascular graft (PMEG) technique in acute aorta disease, even in cases with a previous PMEG requiring a new repair.
Technique: A 77-year-old man presented with an infectious native aortic aneurysm (INAA), which was treated with a PMEG containing fenestrations for the renal arteries and superior mesenteric artery (SMA). After 4 months, a new infectious aneurysm developed at the right renal hilum, which was treated by occluding the right renal artery with a vascular plug.

Background: The details regarding the pathogenesis of hypoxemia in the presence of acute type-A aortic dissection (ATAAD) remains to be fully elucidated. In this study, we investigated the dynamic changes in systemic inflammatory response, gut injury, hypoxemia, and succinate levels in patients with ATAAD and their impact on perioperative hypoxemia.
Methods: We conducted a single-center, observational, case-control study that enrolled 18 patients with ATAAD who underwent emergency total arch repair (TAR) combined with frozen elephant trunk (FET) procedure under hypothermic lower-body circulatory arrest and antegrade cerebral perfusion.
