We report a case of impingement in a metal-on-metal total hip replacement causing both notching of the femoral stem neck and aseptic loosening of the acetabular component. The acetabular component was inserted in excessive anteversion. The femoral stem and acetabular components were retrieved. A larger femoral head was used, and an acetabular component was placed in a less anteverted position. Intra-operative testing through different ranges of movement is recommended to identify potential impingement.

Short-stem total hip arthroplasty (SHA) has become popular because it preserves femoral bone stock and enables the use of short femoral stems in revision total hip arthroplasty (THA). However, no study has evaluated whether a short stem in revision THA, replacing a standard stem, can provide adequate primary stability to facilitate osseous integration. In this biomechanical study, a metaphyseal anchoring SHA (Tri-Lock BPS) stem and a standard THA (Corail) stem were implanted into ten composite femurs and loaded dynamically from 300 to 1700 N with 1 Hz.

Osteoporosis is caused by an imbalance between bone resorption and formation, which decreases bone mass and strength and increases the risk of fracture. Background: Improvements in cementless total hip arthroplasty have been directed at optimizing osseointegration of the femoral implant to reduce aseptic loosening rates.
