Neuropsychiatrists evaluate patients with neurological disorders and general medical conditions who are experiencing psychiatric symptoms. Knowledge of functional neuroanatomy of the central nervous system is essential to offer accurate diagnoses and treatments.

Despite the inclusion of catatonia as a specifier of autism spectrum disorder in DSM-5, we-a team of child and adolescent neuropsychiatrists who specialise in paediatric catatonia and neurodevelopmental disorders-have identified a number of issues with the diagnosis and clinical management of catatonia in our patients. In this Personal View, we summarise the literature regarding catatonia in people with neurodevelopmental disorders, including autism spectrum disorder, describe our concerns, and offer a novel approach to addressing important issues with current diagnostic and treatment paradigms. We emphasise the need for a measure to diagnose and monitor people with catatonia and their history of neurodevelopmental disorders.

Background: In 1874, the German neuropsychiatrist Richard von Krafft-Ebing (RKE) published one of the most influential monographs on melancholia in the latter half of the 19th century entitled "Melancholia: A Clinical Study." This monograph has not been translated into English nor extensively discussed in the modern Anglophonic literature.

Results: The monograph has three chapters describing, respectively, Melancholia without Delusions, Melancholia with precordial anxiety and Psychotic Melancholia.

Background: A 2011 survey of neurologists' attitudes to conversion disorder found a tacit acceptance of the psychological model but significant ambivalence around its relationship to feigning. These issues are under increased scrutiny as the DSM-5 revision removed both the requirement for a psychological formulation and the exclusion of feigning from the diagnostic criteria. Whether those attitudes are shared with psychiatrists is unknown.
