Clinical research of Chinese medicine, due to its own features, is not suitable to be studied by randomized controlled trial methods in some cases. The single-arm clinical trials with objective performance criteria could be taken as a more ideal alternative method. In this paper the basic principles of single-arm clinical trials with objective performance criteria was introduced. Correlated problems of the selection of therapeutic efficacy index, the target identification, case standards, sample size calculation, statistical inference, and interpretation of results were also addressed. This method was scientific and feasible. It can partially solve some problems in the clinical research of Chinese medicine. It has better application prospect.

Gut microbiota disruptions after allogeneic hematopoietic cell transplantation (alloHCT) are associated with increased risk of acute graft-versus-host disease (aGVHD). We designed a randomized, double-blind placebo-controlled trial to test whether healthy-donor fecal microbiota transplantation (FMT) early after alloHCT reduces the incidence of severe aGVHD. Here, we report the results from the single-arm run-in phase which identified the best of 3 stool donors for the randomized phase.

Objective: This study aims to quantitatively evaluate the efficacy and safety of various treatment regimens for treatment-resistant depression (TRD) across oral, intravenous, and intranasal routes to inform clinical guidelines.
Methods: A systematic review identified randomized controlled trials on TRD, with efficacy measured by changes in the Montgomery-Åsberg Depression Rating Scale (MADRS). We developed pharmacodynamic and covariate models for different administration routes, using Monte Carlo simulations to estimate efficacy distribution.
