Introduction: Extubation failure after neonatal cardiac surgery is associated with increased intensive care unit length of stay, morbidity, and mortality. We performed a quality improvement project to create and implement a peri-extubation bundle, including extubation readiness testing, spontaneous breathing trial, and high-risk criteria identification, using best practices at high-performing centers to decrease neonatal and infant extubation failure by 20% from a baseline of 15.7% to 12.

: Female infertility due to unexpected causes exhibits a great challenge for both clinicians and women who are trying to conceive. The present clinical case series study aimed to evaluate the efficacy of multiple wavelengths of red and near-infrared (NIR) laser photobiomodulation (PBM) for increasing the potential of fertility in women and improving reproductive health in unexplained infertility issues. The objectives were to assess the following: (1) any adverse effects; (2) the possibility of producing an effective PBM protocol; (3) and healthy live birth.

Advances in genome editing technology have made it possible to create genome-edited (GE) animals, which are useful for identifying isolated genes and producing models of human diseases within a short period of time. The production of GE animals mainly relies on the gene manipulation of pre-implantation embryos, such as fertilized eggs and two-cell embryos, which can usually be achieved by the microinjection of nucleic acids, electroporation in the presence of nucleic acids, or infection with viral vectors, such as adeno-associated viruses. In contrast, GE animals can theoretically be generated by fertilizing ovulated oocytes with GE sperm.

Article Synopsis
  • - The use of non-steroidal anti-inflammatory drugs targeting PTGS enzymes may contribute to miscarriage, with reduced PTGS2-derived prostaglandins linked to implantation failures in patients.
  • - In mouse models, the role of PTGS2 during pregnancy is debated, with some studies suggesting its involvement in ovulation only while others point to broader effects on fertilization and implantation.
  • - Our research indicates that PTGS2 in the uterine epithelium does not impact early pregnancy events, but is crucial for post-implantation processes such as embryo chamber formation, with significant implications for understanding embryo viability.

Septins as key players in spermatogenesis, fertilisation and pre-implantation embryogenic cytoplasmic dynamics.

Septins are a family of cytokinesis-related proteins involved in regulating cytoskeletal design, cell morphology, and tissue morphogenesis. Apart from cytokinesis, as a fourth component of cytoskeleton, septins aid in forming scaffolds, vesicle sorting and membrane stability. They are also known to be involved in the regulation of intracellular calcium (Ca) via the STIM/Orai complex.
