A 48-year-old man presented with a 2-month history of polyuria, polydypsia, chest pain, fever, cough and extreme weight loss. He was diagnosed with diabetic ketoacidosis and investigations revealed widespread infection with an empyema complicated by bronchopleural fistula, and iliopsoas, suprapubic and periarticular abscesses. Streptococcus milleri was cultured from all sites. A multidisciplinary medical and surgical approach was required for treatment. This case highlights the immunosuppression, and life-threatening complications arising from undiagnosed diabetes mellitus.

Introduction And Importance: Ureteric stump syndrome [USS] is a series of febrile recurrent lower abdominal pain, urinary tract infections, and hematuria that sometimes present with empyema as a rare complication. The ureteric stump is left after ureteric re-implantation due to an impacted stone at the Vesical-ureteric junction [VUJ], or after nephrectomy of a non-functional kidney due to a distal stone; the ureteral stump forms a source of infection to the urinary bladder, in addition to long-term obstructive stones left at the ureteric stamp. This usually cause chronic irritation of the mucosa and potentially change to metaplasia, dysplasia and malignancy.

Background: Blastomycosis is a disease caused by the fungus Blastomyces-a thermally dimorphic fungus that can cause granulomatous and/or purulent infection.
Case Presentation: We report here a case of chronic blastomycosis infection in a 24-year-old male patient from Saudi Arabia who presented with recurrent skin abscesses associated with deep-seated and multilevel paraspinal (dorsal and lumbar) collections and bilateral empyema with pulmonary involvement and bilateral psoas abscesses. The diagnosis was made after a CT-guided pleural biopsy revealed the characteristic histopathological findings of blastomycosis.
A 43-year-old male patient with underlying diabetes mellitus presented with lower abdominal pain radiating to left thigh. He was hemodynamically stable, but appeared lethargic. Both iliac fossae were tender, and crepitus appreciated along the anteromedial aspect of the left thigh.
