Published: February 2013

Background: pulmonary insufficiency after transannular patch repair of tetralogy of Fallot is associated with a poorer outcome.

Objective: to compare the results of 2 techniques of transannular patch repair.

Methods: 93 patients with tetralogy of Fallot and hypoplasia of the right ventricular outflow tract underwent repair between 1990 and 2004 by 2 different techniques. Their ages ranged from 6 months to 12 years. The conventional transannular patch repair was used in 38 patients and 55 had transannular patch repair with pulmonary cusp augmentation.

Results: mortality rates were 13.16% after conventional transannular patch repair and 0% after transannular patch repair with pulmonary cusp augmentation. Follow-up periods were 13-18 years (mean, 15.7±1.64 years) and 5-14 years (mean, 10.8±2.07 years), respectively. The early and midterm results demonstrated significantly more pulmonary insufficiency in the conventional repair group. Transannular patch repair with pulmonary cusp augmentation had longer bypass and operative times, but it was associated with a lower operative mortality than conventional repair. The long-term results showed differences in pulmonary insufficiency, cardiothoracic ratio, and rate of reoperation for pulmonary valve replacement.

Conclusion: reconstruction of the native pulmonary valve accompanied by pulmonary cusp augmentation can reduce postoperative pulmonary insufficiency and has a more favorable long-term outcome.

Objectives: To describe the workflow and value of three-dimensional rotational angiography (3DRA) in percutaneous pulmonary valve implantation (PPVI).

Background: 3DRA offers visualization of the entire topography in the chest and may enhance safety and reduce the risk for complications in PPVI through improved pre-procedural planning and per-procedural guidance.

Methods: All PPVI procedures with the use of 3DRA performed between August 2011 and December 2022 were reviewed.

Association of Ebstein's anomaly of tricuspid valve (TV) with tetralogy of Fallot (TOF) is extremely rare. Their coexistence is unique as they modify each other's physiology. We report a case of a girl child having this rare combination along with Diamond-Blackfan syndrome awaiting a bone marrow transplant.

Unlabelled: Tetralogy of Fallot (TOF) repair involves the placement of a transannular patch (TAP) to relieve right ventricular outflow tract (RVOT) obstruction. TAP results in free pulmonary regurgitation (PR) after surgery. PR is responsible for most of the long-term complications in patients with operated TOF.

Article Synopsis
  • Early recovery after surgery for congenital heart disease (specifically Tetralogy of Fallot/pulmonary stenosis) is gaining focus, as it has favorable long-term outcomes.
  • A study analyzed data from 249 patients from January 2015 to December 2022 to identify factors that impact early extubation following surgery.
  • Results indicated that valve sparing repairs led to quicker recovery compared to transannular patch repairs, and the Brixia score may be a better measure for assessing post-operative lung status than traditional indicators.

Objectives: This study primarily aimed to compare the mechanical ventilation durations between pulmonary valve-sparing and transannular patch repair techniques in the surgical correction of Tetralogy of Fallot. Secondary objectives included comparison of demographic characteristics, cardiopulmonary bypass parameters, postoperative vasoactive inotrope requirements, incidence of cardiac conduction abnormalities, echocardiographic findings, intensive care unit and hospitalisation durations, reoperations rates, morbidity and mortality between the 2 approaches.

Methods: This retrospective cohort study was conducted at the Royal Hospital, Muscat, Oman, between January 2016 and December 2019.
