Background: Placental alkaline phosphatase (PLAP) in cerebrospinal fluid (CSF) has been proposed as a tumor marker for intracranial germinomas. The purpose of the present study was to develop a sensitive assay for measuring CSF PLAP and to evaluate the clinical significance of PLAP in patients with germinomas.

Methods: A chemiluminescent enzyme assay for PLAP was developed using an anti-human-PLAP monoclonal antibody. PLAP concentrations were determined in 37 controls, 36 germinomas, 3 nongerminomatous germ cell tumors, 21 gliomas and 12 other brain tumors.

Results: The assay detection limit was 5 pg/ml. The median PLAP concentration in the control group was below the detection limit. Significantly higher PLAP levels were detected in all 36 germinoma patients, with values ranging from 16 to 3,700 pg/ml. The high PLAP concentrations of 17 germinoma patients decreased to below the detection limit after complete remission had been achieved with radiochemotherapy. The sensitivity and specificity of PLAP for germinomas were 94 and 97%, respectively, with a cutoff value of 30 pg/ml.

Conclusions: The results of this study suggest that the determination of CSF PLAP by the chemiluminescent method described here provides a clinically useful tumor marker for the diagnosis and monitoring of intracranial germinomas.

Article Synopsis
  • * A case study reports a 14-year-old boy diagnosed with diabetes insipidus, initially thought to have a high-grade glioma in the left frontal region, but was later confirmed to have a germinoma through histopathology.
  • * Post-surgery, the patient experienced improvement in his hypernatremia and successfully managed his condition with a small dose of desmopressin, highlighting the unique presentation of this germinoma without typical involvement in key brain regions.

Risk-Stratified Radiotherapy in Pediatric Cancer.

Article Synopsis
  • Despite significant improvements in the cure rate of childhood cancer over recent decades, survivors often face late effects, particularly from radiotherapy.
  • Recent strategies aim to reduce or eliminate radiotherapy in children who respond well to chemotherapy, as seen in conditions like Hodgkin lymphoma and certain Wilms tumors.
  • Innovations like molecular subtyping in medulloblastoma and targeted dose escalation in rhabdomyosarcoma and Ewing sarcoma are being explored to enhance treatment outcomes while minimizing late toxicity.

An eight-year-old child presenting with increased thirst, raised intracranial tension and visual deterioration was diagnosed with synchronous suprasellar and pineal lesions, for which she underwent partial resection of the suprasellar lesion. Histopathological examination suggested pure germinoma. Tumor marker evaluation showed significantly raised levels of beta human chorionic gonadotropin (βHCG), favoring a non-germinoma germ cell tumor (NGGCT), leading to a diagnostic dilemma as the histology and βHCG levels were contradictory.

Article Synopsis
  • Hypogonadotropic hypogonadism can result from brain tumors, like germ cell tumors, requiring chemotherapy and radiation for treatment.
  • Two cases of men (ages 26 and 30) with this condition related to a germinoma highlight the importance of gonadotropin replacement therapy to support fertility before cancer treatment.
  • Close collaboration with oncologists is crucial, as they may advise delaying cancer treatment to prioritize fertility, particularly since chemotherapy can further harm sperm production.
