Hyponatremia is a clinically relevant disorder. Ten to twenty per cent of patients in hospitals are affected by it. Hyponatremia is found to occur in almost every clinical department. Recently, vasopressin antagonists have been licensed to treat hyponatremia of the syndrome of inappropriate antidiuretic hormone secretion (SIADH). They provide physicians with the first specific and effective therapy of this hyponatremia. This opens up new avenues for clinical research into the symptoms, findings and consequences of hyponatremia.

Fatal familial insomnia (FFI) is a rare autosomal dominant neurodegenerative disorder characterized by rapidly progressive dementia, severe sleep disturbances, and autonomic dysfunction. The clinical manifestations of FFI can exhibit substantial variations, making it crucial to rule out other conditions, such as autoimmune encephalitis and Creutzfeldt-Jakob disease, during early diagnosis. In this study, we describe the case of a 58-year-old man who experienced persistent insomnia, autonomic symptoms, gait instability, and rapidly progressive dementia.

Unlabelled: Hyponatremia is the most common hydroelectrolyte disorder in hospitalized patients. It is unclear whether there are differences between severe hyponatremia (<125 mEq/L) and very severe hyponatremia (<115 mEq/L) in terms of etiology, response to therapy, and mortality.
Aim: Describe the etiology, symptoms, response to treatment and mortality of hospitalized adults with severe and very severe hyponatremia.
