AI Article Synopsis

  • - We assessed how accurate and consistent three clinical decision rules are for diagnosing strep throat, using oropharyngeal swab culture as the standard reference.
  • - The Abu Reesh rule demonstrated the highest sensitivity, meaning it was better at correctly identifying true positives for strep throat, while the World Health Organization rule had the highest specificity, effectively minimizing false positives.
  • - The variability in how different clinicians interpreted these rules suggests that more training is necessary for healthcare providers.

Article Abstract

We evaluated the accuracy and interobserver variation of 3 clinical decision rules for streptoccocal pharyngitis diagnosis. Oropharyngeal swab culture was the reference. The Abu Reesh rule had the highest sensitivity and the World Health Organization rule showed the highest specificity. The interobserver variation of those rules indicates the need for better training of clinicians.

Background: Radiotherapy with or without concurrent chemotherapy is a standard of care treatment for patients with head and neck squamous cell carcinoma (HNSCC). Upon completion, patients are referred for a post-treatment F-FDG PET/CT (Fluorine-18 fluorodeoxyglucose positron emission tomography/computed tomography) scan to help guide ongoing management by assessing for the presence or absence of residual or recurrent disease and differentiating this from post-treatment inflammation. To improve objective reporting of response, we developed the Christie score.

Quality Assurance of Depression Ratings in Psychiatric Clinical Trials.

J Clin Psychopharmacol

December 2024

From the NRx Pharmaceuticals, Inc, Wilmington, DE.

Background: Extensive experience with antidepressant clinical trials indicates that interrater reliability (IRR) must be maintained to achieve reliable clinical trial results. Contract research organizations have generally accepted 6 points of rating disparity between study site raters and central "master raters" as concordant, in part because of the personnel turnover and variability within many contract research organizations. We developed and tested an "insourced" model using a small, dedicated team of rater program managers (RPMs), to determine whether 3 points of disparity could successfully be demonstrated as a feasible standard for rating concordance.

Background: Manual analysis of histopathological images is often not only time-consuming and painstaking but also prone to error from subjective evaluation criteria and human error. To address these issues, we created a fully automated workflow to enumerate jejunal crypts in a microcolony survival assay to quantify gastrointestinal damage from radiation.

Methods And Materials: After abdominal irradiation of mice, jejuna were obtained and prepared on histopathologic slides, and crypts were counted manually by trained individuals.

A comprehensive guide to study the agreement and reliability of multi-observer ordinal data.

BMC Med Res Methodol

Faculty of Health Sciences, Oslo Metropolitan University, P.O box 4 St Olavs plass, Oslo, N-0130, Norway.

Background: A recent systematic review revealed issues in regard to performing and reporting agreement and reliability studies for ordinal scales, especially in the presence of more than two observers. This paper therefore aims to provide all necessary information in regard to the choice among the most meaningful and most used measures and the planning of agreement and reliability studies for ordinal outcomes.

Methods: This paper considers the generalisation of the proportion of (dis)agreement, the mean absolute deviation, the mean squared deviation and weighted kappa coefficients to more than two observers in the presence of an ordinal outcome.
