Uncovering the Metabolic Footprint of New Psychoactive Substances by Metabolomics: A Systematic Review.

Molecules

January 2025

Laboratório de Química Orgânica e Farmacêutica, Departamento de Ciências Químicas, Faculdade de Farmácia, Universidade do Porto, Rua Jorge Viterbo Ferreira, 228, 4050-313 Porto, Portugal.

New psychoactive substances (NPSs) emerged in the 2000s as legal alternatives to illicit drugs and quickly became a huge public health threat due to their easy accessibility online, limited information, and misleading labels. Synthetic cannabinoids and synthetic cathinones are the most reported groups of NPSs. Despite NPSs being widely studied, due to their structural diversity and the constant emergence of novel compounds with unknown properties, the development of new techniques is required to clarify their mode of action and evaluate their toxicological effects.

Wastewater analysis technology has emerged as a promising tool for monitoring illicit drug consumption. However, the current reliance on the solid-phase extraction (SPE) pre-treatment method presents significant challenges for widespread adoption and high-throughput monitoring, as it consumes a large amount of time and labor as well as requires specialized instruments. This study has developed a direct injection (DI) technique for UPLC-MS/MS, enabling the detection of 77 compounds encompassing metabolites of human biomarkers, illicit drugs, and new psychoactive substances.

Article Synopsis
  • A study was conducted in Danish prisons to evaluate an initial on-site drug screening method and understand drug trends among inmates over two years.
  • The analysis involved urine samples from 710 inmates, testing for 123 substances using both on-site immunoassays and confirmatory lab methods, revealing high sensitivity and specificity in the screening process.
  • While the immunoassay was effective, it missed several significant drugs, leading to plans for a more comprehensive testing approach to reduce false negatives in detecting drug use.

A critical review of wastewater-based epidemiology as a tool to evaluate the unintentional human exposure to potentially harmful chemicals.

January 2025

Department of Environmental Chemistry, Institute of Environmental Assessment and Water Research (IDAEA), Severo Ochoa Excellence Center, Spanish Council of Scientific Research (CSIC), Jordi Girona 18-26, E-08034, Barcelona, Spain.

Wastewater-based epidemiology (WBE) is a powerful tool to gather epidemiological insights at the community level, providing objective data on population exposure to harmful substances. A considerable portion of the human exposure to these potentially harmful chemicals occurs unintentionally, unlike substances such as pharmaceuticals, illicit drugs, or alcohol. In this context, this comprehensive review analyzes WBE studies focused on classes of organic chemicals to which humans are unintentionally exposed, namely organophosphorus flame retardants, per- and polyfluoroalkyl substances (PFAS), benzotriazoles and benzothiazoles, phthalates and terephthalates, benzophenones, pesticides, bisphenols, and parabens.

Article Synopsis
  • The study evaluated how effective a screening program for liver fibrosis is in reducing alcohol consumption among individuals with alcohol use disorder (AUD) at the Hospital Clinic of Barcelona.
  • Out of 334 individuals screened, 45% were abstinent from alcohol after 6 months, significantly higher than the 29% abstinence rate in a control group.
  • Key factors promoting abstinence included receiving medication for AUD, no illicit drug use, and participation in the screening program, highlighting the benefits of early diagnosis and lifestyle counseling.
