Evolution of covered stents in the contemporary era: clinical application, materials and manufacturing strategies using nanotechnology.

Endovascular stents have revolutionised the field of interventional cardiology. Despite their excellent clinical outcome complications associated with percutaneous stent implantation following the procedure have remained a major drawback in their widespread use. To overcome such limitations, a number of novel endovascular stents have emerged including a covered stent wrapped in a thin membrane sleeve. As well as prevention of complications associated with stenting, covered stents owing to their physical barrier are used as the treatment option of choice for trauma devices during emergency situations and to treat a number of pathological disease states. The aim of this review is to provide the reader with an overall objective outlook in the use of covered stents as a treatment option in a number of vascular complications and addresses their design and materials used in the manufacturing process. In addition, new strategies are highlighted and future prospects with the emergence of novel smart alloys for 3D scaffolds and the use of nanotechnology in the development of nanocomposite materials.

Ascending aortic pseudoaneurysm may pose significant risk for reoperative repair. We describe an 18-year-old man who presented with bacteremia and a large, mycotic ascending aortic pseudoaneurysm 3 months after redo cardiac surgery. A covered stent graft sealed the pseudoaneurysm neck and facilitated safe reentry into the mediastinum.

A 54-year-old man underwent right S6 segmentectomy for right lung cancer. After discharge, he presented with fever, hemoptysis, and cough, and computed tomography showed an intermediate bronchus fistula. Because direct closure or bronchoplasty was challenging, a Dumon (Novatech) stent was inserted directly into the fistula from the surgical field and covered with an autologous pericardial patch, pedicled mediastinal fat, and intercostal muscle.

Purpose: The AVeNEW Post-Approval Study (AVeNEW PAS) follows upon results from the AVeNEW IDE clinical trial and was designed to provide additional clinical evidence of safety and effectiveness using the Covera™ Vascular Covered Stent to treat arteriovenous fistula (AVF) stenoses in a real-world hemodialysis patient population.

Materials And Methods: One hundred AVF patients were prospectively enrolled at 11 clinical trial sites in the USA and treated with the covered stent after angioplasty of a clinically significant target stenosis. The primary safety outcome was freedom from any adverse event that suggests the involvement of the AV access circuit evaluated at 30 days.

Objectives: Mal-deployment of the thoracic endovascular aortic repair (TEVAR) stent graft during a frozen elephant trunk (FET) procedure for an acute type A aortic dissection (ATAAD) leads to devastating complications. We report a hemiarch replacement with TEVAR stent graft covering the aortic arch vessels salvaged through an endovascular approach.

Methods: A 69-year-old man with ATAAD in 2018, status post-hemiarch repair with TEVAR, presented in 2023 with progressive dizziness/syncope and lower extremity hypertension with inability to tolerate anti-hypertensives.
