Patients on long-term steroid therapy are often given supplementary intravenous glucocorticosteroids during perioperative care. The reason is to minimize the risk of acute adrenal insufficiency, which is a potential life threatening condition. Based on a recent Cochrane review and the available literature on the subject a rational way in dealing with the matter is described.

Background: Brain metastases are a common complication in patients with non-small-cell lung cancer (NSCLC) lacking actionable driver mutations, with limited treatment options and poor prognosis. We aimed to investigate the efficacy and safety of brain radiotherapy combined with camrelizumab and platinum-doublet chemotherapy in patients with newly diagnosed advanced NSCLC and brain metastases.
Methods: This multicentre, single-arm, phase 2 trial was done across nine tertiary hospitals in China.

Background: Intermediate-high risk pulmonary embolism (PE) carries a significant risk of hemodynamic deterioration or death. Treatment should balance efficacy in reducing clot burden with the risk of complications, particularly bleeding. Previous studies on high-dose, short-term thrombolysis with alteplase (rtPA) showed a reduced risk of hemodynamic deterioration but no change in mortality and increased bleeding complications.
