The current situation for patients with nervous intractable diseases is that there are few places where they can be hospitalized for a long term, and their family members bear a heavy burden for caring for them. In order to support home care, our hospital is formulating a system in which regular respite hospitalization is possible, and we are supporting home care of many patients with nervous intractable diseases.

Nanobodies play an increasingly prominent role in cancer imaging and therapy. However, their efficacy is often constrained by inadequate tumor penetration and rapid clearance from the bloodstream, particularly in brain tumors due to the intractable blood-brain barrier (BBB). Glycosylation is a favorable strategy for modulating the biological functions of nanobodies, including permeability and pharmacokinetics, but it also leads to heterogeneous glycan structures, which affect the targeting ability, stability, and quality of nanobodies.

Neuroimmunology is reshaping the understanding of the central nervous system (CNS), revealing it as an active immune organ rather than an isolated structure. This review delves into the unprecedented discoveries transforming the field, including the emerging roles of microglia, astrocytes, and the blood-brain barrier (BBB) in orchestrating neuroimmune dynamics. Highlighting their dual roles in both repair and disease progression, we uncover how these elements contribute to the intricate pathophysiology of neurodegenerative diseases, cerebrovascular conditions, and CNS tumors.

The World Health Organization in 2021 ranked Alzheimer's disease and other dementias as the seventh leading cause of death globally. Neurodegenerative disorders are progressive, intractable, and often fatal diseases. Early diagnosis may allow patients to enjoy prolonged survival with attenuated symptomatology because of early intervention.

Rosacea is a chronic inflammatory disease primarily affecting the central facial region, significantly involving the facial blood vessels and the sebaceous gland units associated with hair follicles. The stellate ganglion block (SGB) technique can restore balance to autonomic nervous function by interrupting the impulse conduction of preganglionic and postganglionic sympathetic nerve fibers, thereby alleviating excessive peripheral blood vessel contraction, enhancing tissue blood supply, balancing hormone secretion, and modulating immune responses. SGB has demonstrated remarkable efficacy in treating various skin conditions affecting the head, face, and neck.

The rising prevalence of treatment-resistant neuropsychiatric disorders underscores the need for innovative and effective treatment strategies. The gut microbiota (GM) plays a pivotal role in the progression of these diseases, influencing the brain and mental health through the gut-brain axis (GBA). The vagus nerve plays a significant role in the GBA, making it a key area of focus for potential novel therapeutic interventions.
