Pseudoaneurysm of uterine artery: a rare cause of secondary postpartum hemorrhage.

Objectives: Secondary PPH has received little attention. In more than half of cases the etiology is not made out. Vascular abnormalities like pseudoaneurysm and AV malformation are rare but detectable and easily treatable.

Methods: This is an analysis of five women presenting with severe secondary PPH after the 3rd to 6th week of postpartum. All women were post LSCS with no identifiable cause and referred for further management. They were diagnosed to have pseudoaneurysm by angiography and uterine artery embolisation was done.

Results: Five women were analysed. Four of them had a pseudoaneurysm and were embolised one woman was hemodynamically unstable and hence taken up for hysterectomy.

Conclusion: Pseudoaneurysm is an important vascular abnormality especially following LSCS. Uterine artery embolisation is safe and reliable alternative technique and prevents a hysterectomy.

Objectives: To compare the maternal hemodynamic profile at 12 + 0 to 15 + 6 weeks' gestation in women who subsequently developed pre-eclampsia (PE) and those who did not, and to assess the screening performance of maternal hemodynamic parameters for PE in combination with the Fetal Medicine Foundation (FMF) triple test, including maternal factors (MF), mean arterial pressure (MAP), uterine artery pulsatility index and placental growth factor.

Methods: This was a prospective case-control study involving Chinese women with a singleton pregnancy who underwent preterm PE screening at 11 + 0 to 13 + 6 weeks' gestation using the FMF triple test, between February 2020 and February 2023. Women identified as being at high risk (≥ 1:100) for preterm PE by the FMF triple test were matched 1:1 with women identified as low risk (< 1:100) for maternal age ± 3 years, maternal weight ± 5 kg and date of screening ± 14 days.

Ethnopharmacological Relevance: The classic TCM prescription, Shoutai Wan (STW), is extensively used in clinical settings to manage threatened miscarriage and Recurrent spontaneous abortion (RSA). The complexity of pregnancy physiology, coupled with diverse etiologies, and the specificity of energy metabolism for normal embryo attachment and development,pose challenges to clinical diagnosis and treatment. The specific molecular mechanisms of how STW regulates these biological processes and contributes to the treatment of RSA remain to be elucidated.
