Chronic granulomatous disease is a genetic disorder responsible for a defect in the NADPH oxidase of phagocytic cells. It impairs the oxidative burst necessary to the intracellular inactivation of microorganisms and predisposes to an increased risk of infections by various microorganisms, including fungi like Aspergillus spp. and other less frequently encountered or emerging fungal species. Here we review the genetic basis, pathogenesis and clinical presentation associated with fungal infections in chronic granulomatous disease as well as the current prophylaxis and newly available therapies.

A drug-induced sarcoidosis-like reaction (DISR) is a systemic granulomatous reaction indistinguishable from sarcoidosis and is associated with the administration of a medication. It typically exhibits a temporal relationship with the initiation of the drug (an average interval of 22 months) and tends to improve upon its discontinuation. Tumor necrosis factor (TNF) antagonists, including adalimumab, have been associated with the development of DISR.

We describe a case of sarcoidosis in a previously healthy 39-year-old man with the development of an acute kidney injury, requiring renal replacement therapy, as the first manifestation of the disease. The course of the disease was complicated by a сatheter-associated bloodstream infection.
