This report describes the case of a 43-year-old female patient presenting with left lower quadrant pain and bloody diarrhea, initially managed as infectious colitis. Despite treatment, symptoms persisted, leading to repeat imaging showing mild pancolitis with colon mucosal hyperenhancement and wall thickening. Admission for intravenous hydration and subsequent colonoscopy revealed findings consistent with ischemic colitis.

A 30-year-old female patient had an 8-cm retroperitoneal mass that was incidentally discovered during an abdominal computed tomography (CT) scan performed for diagnosing ischemic colitis, resulting in her referral to our hospital. A CT-guided needle biopsy was performed with a 20G needle, but the results were inconclusive. A 14G needle was used after weighing the benefits and harms, resulting in the diagnosis of a hyaline-vascular variant of Castleman disease.

Colonic obstruction is reported in 85% of emergency colorectal surgery for cancer. Colonic ischemia, however, is a rare entity and is found in 5% of these emergency cases. Background: Cases of Hepatic portal venous gas (HPVG) have been associated with high mortality rates and frequently require emergency exploratory laparotomy. However, the widespread utilization of computed tomography (CT) scans has revealed that HPVG is often connected to benign conditions, as demonstrated by numerous studies. Given the intricate nature of the underlying causes of HPVG, there remains a lack of consensus regarding the necessity of emergency surgical exploration for patients with HPVG.
