Safety of pharmacoenhancers for HIV therapy.

HIV protease inhibitors have been part of highly active therapy regimens throughout the past 16 years of antiretroviral therapy. The majority of currently available protease inhibitors are coadministered with low-dose ritonavir (RTV) as a pharmacoenhancer, significantly increasing their plasma concentrations. This therapeutic principle called 'boosting' helped to overcome the obstacles of this class of drugs concerning unfavorable pharmacokinetics, resulting in either a high frequency of dosing or subtherapeutic plasma concentrations. The coadministration of RTV together with HIV protease inhibitors created a potent, clinically effective antiretroviral therapy, which is more convenient for the patient, and has revealed a high genetic barrier to viral resistance. RTV is the only pharmacoenhancing agent available to date; however, cobicistat, a selective CYP3A4 inhibitor, is about to enter the market. This article reviews current literature, compares the safety of both pharmacoenhancers and appraises their use in current and future HIV therapy.

Objective: To evaluate the clinical characteristics and risk factors of infections occurring during hospitalization in patients with multiple myeloma(MM) treated with new generation therapies (including immuno- modulatory drugs, proteasome inhibitors and monoclonal antibodies).

Methods: The clinical data were collected from 155 patients with multiple myeloma who were treated in Shanxi Bethune Hospital from March, 2017 to March, 2022 and were retrospectively analyzed. For this study, the following therapies were considered to be new generation therapies: lenalidomide, pomadomide, bortezomib, ixazomib, daratumumab.

Heart failure (HF) and renal dysfunction often coexist and interact in many complex and bidirectional pathways, leading to detrimental effects on patient outcomes. The treatment of HF patients with renal dysfunction presents a significant clinical challenge. Interestingly, sacubitril/valsartan, an angiotensin receptor-neprilysin inhibitor (ARNI), may have beneficial effects on cardiac and renal outcomes in patients with HF with reduced ejection fraction, particularly by slowing the rate of decrease in the estimated glomerular filtration rate compared to a single angiotensin-converting enzyme inhibitor.

Current anti-HIV drugs have significantly improved the prognosis of HIV infected patients so much so that it is now considered a chronic disease, and adherence to medications keeps non-detectable amounts of the virus in the body.
