Unlabelled: The aim of this study was the determination of the particularities of the cardiac remodeling with echo-cardio graphic methods at patients with chronic heart failure (CHF) secondary to congenital heart diseases and pulmonary arterial hypertension (PAH).

Material And Methods: In the study 204 children were included (average age of 37.1 +/- 3.7 months) with CHF secondary to congenital systemic-to-pulmonary shunts (simple, combined and complex) associated with PAH. Depending on the rang of PAH the children were divided into 2 subgroups: IA--59 patients with CHF and moderate PAH and IB--145 children with CHF and severe PAH. Using the echocardiography 2D the right and left dimensions of the heart, EF, SF of LV were determined; the Doppler exam--the global function of RV (Tei performance index), systolic and mean pressure in PA.

Results And Conclusions: The particularities of the cardiac remodeling estimated echocardiography with diagnostic value at patients with CHF secondary to congenital shunts complicated with PAH are the following: dilatation of the right cavities of the heart (RVDd and RAD) and of the PA with the tendency of reduction of the LV dimensions, hypertrophy of the anterior wall of the RV, the increasing of the diameter of the fibrosis ring at tricuspid valve, the global dysfunction of RV (Tei index), valvular insufficiency (TVI and PVI), the diminishing of the cardiac index. These parameters were more significant at patients with severe PAH (p < 0.001), denoting the consequences of the pressure overload on the cardiac remodeling, estimating the level of hemodynamic severity and with certainty can have a special meaning in deciding the therapeutic tactics, prognosis and evolution.

Background: Despite antihypertensive treatment, some high-risk hypertensive patients still experience major adverse cardiovascular events (MACEs). Current risk stratification tools may underestimate the presence of metabolites in hypertension and thereby risk of MACEs.

Objectives: We aimed to explore the potential value of gut microbiota-derived metabolite phenylacetylglutamine (PAGln) in risk stratification of hypertension.

The Management of the Aortic Arch in Type A Aortic Dissection: Replace, Repair with the AMDS, or Leave for Another Day?

J Cardiovasc Dev Dis

Division of Cardiac Surgery, Department of Surgery, University of Alberta, Edmonton, AB T6G 2R3, Canada.

Objectives: Acute type A aortic dissection (ATAAD) is a life-threatening condition that requires emergent surgical intervention. Numerous surgical approaches exist for ATAAD, and controversy remains regarding the optimal arch interventions for ATAAD patients. Aortic Arch Interventions: Approaches to ATAAD repair include hemiarch repair or extended arch repairs, including the hemiarch with a hybrid stent implantation, such as the AMDS hybrid Prosthesis, total arch replacement (TAR), and the use of an elephant trunk and frozen elephant trunk.

Currently, there are no therapies that prevent the negative myocardial remodeling process that occurs after a heart attack. Injectable hydrogels are a treatment option because they may replace the damaged extracellular matrix and, in addition, can be administered minimally invasively. Reactive oxygen species generated by ischemia-reperfusion damage can limit the therapeutic efficacy of injectable hydrogels.

Decoding the Epigenetic and Transcriptional Basis of Direct Cardiac Reprogramming.

Department of Biomedical Engineering, Heersink School of Medicine, School of Engineering, University of Alabama at Birmingham.

Heart disease, particularly resulting from myocardial infarction (MI), continues to be a leading cause of mortality, largely due to the limited regenerative capacity of the human heart. Current therapeutic approaches seek to generate new cardiomyocytes from alternative sources. Direct cardiac reprogramming, which converts fibroblasts into induced cardiomyocytes (iCMs), offers a promising alternative by enabling in situ cardiac regeneration and minimizing tumorigenesis concerns.
