An unrecognized case of Acanthamoeba keratitis became quiescent after prolonged scleritis, resulting in a central corneal scar with extensive scleral ectasia. Twenty-one months after the onset of the sclerokeratitis, a penetrating keratoplasty was performed. Acanthamoeba cysts were found in the host corneal button. The corneal transplant has remained thin and clear for 2 1/2 years following surgery. Acanthamoeba keratitis extending to the limbus may become self-limited due to immunologic mechanisms available at the limbus, which do not appear to be active within the cornea itself. However, the prolonged inflammatory reaction manifesting as scleritis may result in extensive scleral ectasia.

Objective: To determine if scleral fittings that occur before corneal cross-linking (CXL) are still successful after the procedure.
Methods: This prospective study included seven patients with keratoconus or post-laser-assisted in situ keratomileusis (LASIK) corneal ectasia who were fitted with scleral lenses then underwent CXL. Four patients (six eyes) had keratoconus and three patients (five eyes) had post-LASIK ectasia.

Purpose: Scleral cross-linking (SXL) with ultraviolet A (UVA) and riboflavin has already been used in laboratory studies for scleral stiffness increase as a potential treatment for progressive myopia and scleral ectasia. This study aims to investigate whether the regional application of scleral cross-linking (SXL) with ultraviolet A (UVA) and riboflavin in fresh porcine eye globes affects the ocular rigidity as well as its impact on intraocular pressure after an induced acute increase in the volume of intraocular fluid.
Methods: The study included two groups of fresh porcine eyes: an experimental group (n=20) that underwent scleral cross-linking (SXL) with riboflavin and UVA applied to the posterior sclera and a control group (n=20) that did not receive SXL treatment.

This systematic review evaluates the long-term effectiveness of scleral lenses in improving visual outcomes, patient satisfaction, and safety in patients with keratoconus. A systematic search of six databases (PubMed, Embase, Web of Science, Scopus, Cochrane Library, and MEDLINE) was performed following Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. The inclusion criteria included studies with more than or equal to three months of follow-up, included a minimum of 20 keratoconus patients, published after year 2020 and outcomes reporting visual acuity, comfort or quality of life.

Purpose: To report interim results of a single center, randomized, double-blind, crossover-controlled clinical trial comparing wavefront-guided (wfg) scleral lenses (SLs) to traditional scleral lenses (tSLs) for patients with a history of irregular corneal astigmatism (ICA).
Methods: Thirty-one ICA eyes of 18 participants were reviewed, 23 with keratoconus, six postcorneal refractive ectasia, and two postpenetrating keratoplasty. Patients with corneal or lens opacities were not excluded from participating.
