Comparison of fetal heart rate patterns using nonlinear dynamics in breech versus cephalic presentation at term.

Background: It has been reported that breech fetuses have inferior neurological outcomes regardless of mode of delivery, raising the possibility that in utero neurological impairment is more frequent in breech fetuses, possibly contributing to malpresentation.

Aims: To assess differences between the cardiovascular autonomic nervous systems (ANSs) of breech and cephalic fetuses using nonlinear dynamic indices of fetal heart rate (FHR) variability.

Study Design And Subjects: This study included 86 fetuses with breech presentation and 173 fetuses with cephalic presentation, with no other maternal or fetal problems. We analyzed FHR variability and spectral indices as markers of ANS behavior. We used nonlinear dynamic indices to represent the complexity of heart rate regulation, as well as correlation dimension as a chaotic index of the cardiovascular control system.

Results: One of FHR parameters (Mean minute range) was significantly lower in breech than cephalic fetuses (p=0.0294). However, there were no other significant differences in any linear or nonlinear indices, nor in clinical outcomes, between breech and cephalic fetuses.

Conclusion: Our data suggest that breech fetuses have neither more active ANS nor less active complexity control systems than do cephalic fetuses. This indicates that the neurologic maturation of breech fetuses is not inferior to cephalic ones. The practical implication of these findings is that the nervous system integrity of breech fetuses may not result directly in neonatal complications.

Vaginal delivery of the second twin: simulation to improve trainee knowledge and comfort.

Minerva Obstet Gynecol

January 2025

Obstetrics and Gynecology Unit, IRCCS Istituto Giannina Gaslini, Genoa, Italy.

Background: Vaginal delivery in twins is feasible but challenging. Successful vaginal delivery of a non-vertex second twin depends on knowledge of specific obstetrical maneuvers. Skill acquisition at the patient's bedside is difficult, making simulation training an integral part of obstetrics and gynecology residency programs.

Background: Like other countries in sub-Saharan Africa, Madagascar has a high burden of maternal and neonatal mortality. However, as the proportion of foetal and placental abnormalities among the Malagasy population is unknown, strategies aimed at reducing maternal and neonatal mortality are challenging to define and implement.

Methods: We conducted a multi-year, cross-sectional study using secondary NGO data on obstetric ultrasound, including patient records of all pregnant women who received an obstetric ultrasound screening between July 1st, 2017, and September 30th, 2020, at 62 public-sector primary care facilities in urban and rural regions of Madagascar.

Article Synopsis
  • The study aimed to evaluate the visibility of the fetal optic chiasm and optic tracts through ultrasound and establish reference values for mid-trimester fetuses.
  • It included 154 normal fetuses at 19-23 weeks of gestation, measuring various dimensions using a specific ultrasound technique, successfully visualizing OC and OTs in 70.8% of cases.
  • Results showed that the dimensions of OC and OTs were positively correlated with measurements of the cavum septum pellucidum, as well as other growth indicators like biparietal diameter and head circumference.

Successful Delayed Interval Delivery in Twin Pregnancy After Early Premature Rupture of Membranes of a Leading Fetus: A Case Report.

Medicina (Kaunas)

November 2024

Department of Obstetrics and Gynecology, Daegu Dongsan Hospital, Keimyung University School of Medicine, 56, Dalseong-ro, Jung-gu, Daegu 41931, Republic of Korea.

A 36-year-old primigravida conceived dichorionic diamniotic twins via in vitro fertilization. The first twin experienced premature rupture of membranes at 18 weeks and 5 days of gestation. Despite antibiotic treatment, the first fetus suffered intrauterine fetal death and was delivered three days later at 19 weeks and 1 day.
