To investigate the human corticofacial projections, we recorded the compound motor potentials and single motor unit potentials evoked by magnetic transcranial stimulation, in the frontalis and lower facial muscles of healthy subjects. Potentials secondary to activation of the corticobulbar tract were contralateral in lower and bilateral in upper facial muscles. Even though the latency of responses was longer than would be expected for direct cortico-motoneuronal connections, these cannot be excluded either for lower or upper facial motoneurons.

A facial appearance of premature aging due to poor bone support of the soft tissues is frequently found in patients with midface hypoplasia. This study was performed to evaluate the changes in the soft tissues of the cheek area in patients subjected to bimaxillary orthognathic surgery. The cheek line angle and length of 27 consecutive patients who underwent bimaxillary surgery, were measured on cone beam computed tomography scans obtained before surgery and at 1 and 12 months after surgery using 3D software.

An optimal planning, achievable thanks to advanced acquisition of 3D images and thanks to the virtual simulation of the intervention via specific software. Anyway, the final result depends also on factors that cannot be totally predicted regarding the settlement of soft tissues on the rigid structures, and a final objective check would be useful to eventually perform some adjustments before to conclude the intervention.
