Spontaneous mesenteric haematoma; diagnosis and management. | LitMetric

Spontaneous mesenteric haematoma; diagnosis and management.

BMJ Case Rep

Department of Emergency Medicine, Royal Free Hospital, London, UK.

Published: August 2012

Spontaneous mesenteric haematomas are rare. They have been reported to be associated with coagulopathies, connective tissue disorders, past trauma, arteriopathy and pancreatitis. However, some cases have been reported in which there is no apparent underlying aetiology. Here we report such a case and we review the literature that discusses optimal diagnosis and management. In this case, spontaneous haemostasis occurred by intra-abdominal tamponade and the regression of the haematoma was monitored with regular imaging.

Association between aortomesenteric angle and symptomatic spontaneous isolated superior mesenteric artery dissection.

Sci Rep

Department of Vascular and Interventional Radiology, Nanjing First Hospital, Nanjing Medical University, No. 68 Changle Road, Nanjing, 210006, China.

The purpose of this study is to investigate the association between the aortomesenteric angle (AMA) and the occurrence and image characteristics of spontaneous isolated superior mesenteric artery dissection (SISMAD). This is a single-centre retrospective case-control study. Between January 1 2013 and December 13 2022, consecutive patients with computed tomography angiography (CTA) confirmed symptomatic SISMAD were included.

Mesenteric hematomas, a rare and clinically significant condition, are typically associated with abdominal trauma, vascular conditions, or surgical complications. Spontaneous mesenteric hematomas, an even rarer subtype, are often seen in patients who are on anticoagulation therapy or have inguinal hernias. In this case report, we discuss an 89-year-old male patient who initially presented with stable vital signs and abdominal pain but was later discovered to have a large, stable mesenteric hematoma on an abdominal computed tomography scan.

Introduction: The occurrence of type II endoleak (T2EL) presents a significant challenge in standard endovascular aneurysm repair (EVAR), with ongoing debate in the literature regarding its optimal management. Although spontaneous resolution has been observed in many cases, intervention is often required to prevent progressive sac enlargement and rupture. Various approaches have been described, including translumbar, transarterial, and transcaval embolization, as well as direct sac puncture.

Flow patterns and classification within Spontaneous Isolated Superior Mesenteric Artery Dissection (SISMAD) are crucial for selecting subsequent treatment options. This study aims to propose a new classification of SISMAD and to propose two corresponding treatment plans based on this new classification. The 3D models of 70 patients with SISMAD were reconstructed and classified into Li types I-V based on morphology, followed by computational fluid dynamics analysis.
