Cathepsin A (CatA) is a serine carboxypeptidase distributed between lysosomes, cell membrane, and extracellular space. Several peptide hormones including bradykinin and angiotensin I have been described as substrates. Therefore, the inhibition of CatA has the potential for beneficial effects in cardiovascular diseases. Pharmacological inhibition of CatA by the natural product ebelactone B increased renal bradykinin levels and prevented the development of salt-induced hypertension. However, so far no small molecule inhibitors of CatA with oral bioavailability have been described to allow further pharmacological profiling. In our work we identified novel β-amino acid derivatives as inhibitors of CatA after a HTS analysis based on a project adapted fragment approach. The new inhibitors showed beneficial ADME and pharmacokinetic profiles, and their binding modes were established by X-ray crystallography. Further investigations led to the identification of a hitherto unknown pathophysiological role of CatA in cardiac hypertrophy. One of our inhibitors is currently undergoing phase I clinical trials.

Posttransplant diabetes mellitus (PTDM) is associated with significant morbidity and mortality in liver transplant recipients (LTRs). We used the Organ Procurement and Transplantation Network (OPTN) database to compare the incidence of developing PTDM across the United States and develop a risk prediction model for new-onset PTDM using OPTN region as well as donor-related, recipient-related, and transplant-related factors. All US adult, primary, deceased donor, LTRs between January 1, 2007, and December 31, 2016, with no prior history of diabetes noted, were identified.

Purpose: To describe a novel surgical approach for treating patients diagnosed with severe idiopathic epiretinal membranes (iERM) featuring ectopic intrafoveal layers (EIFL).
Method: A retrospective, interventional case series was conducted involving eight cases of iERM with EIFL. The technique involved a 27-gauge vitrectomy, removal of all vitreous adhesions, and injection of approximately 100μl sub-retinal balanced salt solution (BSS) using a 41-gauge subretinal needle, creating 3-4 localized bleb-like elevations around the folded retina to loosen it.

High-velocity lateral impacts to the nose sometimes cause nasal buckle-out fractures with a trapdoor buckle-out segment displaced outwards. Prolonged immobilization of a reduced buckle-out segment at risk for outward redisplacement remains challenging. Here we introduce a novel method of intranasal outer cortex splinting with a Kirshner (K)-wire to reinforce the reduced state and prevent outward re-displacement of the buckle-out segment.
