A case of severe Pseudomonas perichondritis following a 'fashionable' ear-piercing procedure, performed high on the pinna, is reported. The current vogue for such 'high' ear-piercing, which traverses cartilage rather than the fatty tissue of the ear lobe as in 'traditional' ear-piercing, increases the risk of infection which may produce severe cosmetic deformity.

In September 2023, the UK Health Security Agency's (UKHSA) South West Health Protection Team received notification of patients with perichondritis. All five cases had attended the same cosmetic piercing studio and a multi-disciplinary outbreak control investigation was subsequently initiated. An additional five cases attending the same studio were found.

It is not uncommon for patients to present to the emergency department with complications resulting from body piercings. A 57-year-old male chronic smoker with underlying diabetes mellitus presented with dysphonia associated with cough, dysphagia and reduced effort tolerance of 3 months' duration. Videoendoscope finding revealed bilateral polypoidal and erythematous true and false vocal fold with small glottic airway. The patient was initially treated as having tuberculous laryngitis and started on antituberculous drug.

Purpose: Pseudomonas aeruginosa is commonly found in suppurative perichondritis, but the significant pathogens in non-abscess perichondritis are unclarified. We aimed to explore the bacterial findings and evaluate the effectiveness of different antibiotic regimes in the treatment of patients with perichondritis.
Methods: All patients treated for perichondritis at the two Ear-Nose-Throat Departments in Central Denmark Region (covering 20% of the Danish population) from January 1990 to October 2018 were included.
