Fibrosing mediastinitis is a rare chronic inflammatory condition characterized by excessive fibrotic process in the mediastinum. Although various infectious processes are more common causes, it can also be related to several malignancies. We report a case of a 28-year-old woman with fibrosing mediastinitis related to an aggressive primary gray-zone lymphoma causing complete occlusion of the superior vena cava (SVC) and the innominate veins.

Structural abnormalities within the right ventricular outflow tract (RVOT) can present similarly to Brugada syndrome. A 34-year-old woman with no medical history presented with polymorphic ventricular tachycardia/ventricular fibrillation cardiac arrest and initial electrocardiogram showed type I Brugada pattern. Cardiac magnetic resonance imaging revealed prominent tissue thickening at the RVOT with late gadolinium enhancement.

Undifferentiated cardiac tumors represent a diagnostic dilemma that requires meticulous workup. We present the case of a 67-year-old woman with a right atrial mass that was investigated with multiple modalities before being diagnosed as a cardiac lymphoma. The role of transvenous and/or percutaneous biopsy for guiding diagnosis and management is emphasized.

An 80-year-old woman with a history of B-cell non-Hodgkin lymphoma presented to the emergency department with exertional dyspnea and lower limb edema. A transthoracic echocardiogram revealed a large extracardiac mass invading the right atrium. A diagnostic transcatheter endomyocardial biopsy guided by intracardiac echocardiography was performed.

This case highlights the prompt use of cardiac computer tomography angiography in a patient with intracardiac localization of diffuse large B-cell lymphoma and chest pain in the acute setting. Information on anatomical associations of the metastases with the coronaries serves as the foundation for subsequent therapeutic and diagnostic strategies.
