The case of a 66-year-old female who developed sudden redness and swelling of her left thumb is presented. A biopsy showed well formed granulomas, mixed inflammation with a predominance of neutrophils, and acid-fast bacilli. After negative culture results, tissue was sent for a broad-range polymerase chain reaction amplification followed by suspension array identification, which classified the pathogenic organism as Mycobacterium avium-intracellulare. The patient was started on clarithromycin, rifabutin and ethambutol leading to clinical improvement. M. avium-intracellulare is present in soil, fresh water, sea water, dairy products and some animal tissues. Primary cutaneous manifestations are uncommon in the immunocompetent host. As cultures may be negative or can take up to three weeks to show growth, the molecular approach described here offers an opportunity for more rapid and specific diagnosis.

Nanozymes with Modulable Inhibition Transfer Pathways for Thiol and Cell Identification.

The elementary mechanism and site studies of nanozyme-based inhibition reactions are ambiguous and urgently require advanced nanozymes as mediators to elucidate the inhibition effect. To this end, we develop a class of nanozymes featuring single Cu-N catalytic configurations and B-O sites as binding configurations on a porous nitrogen-doped carbon substrate (B/Cu) for inducing modulable inhibition transfer at the atomic level. The full redistribution of electrons across the Cu-N sites, induced by B-O sites incorporation, yields B/Cu with enhanced peroxidase-like activity versus Cu.

Herein, the lanthanide "personality function tailoring" HOF composite sensor array is designed for the specific discrimination of biothiols (GSH, Cys, and Hcy) based on the activation of various luminescent molecules, such as r-AuNCs/luminol via HOF surface proximity. Lumi-HOF@Ce serves as a versatile platform for catalyzing the oxidation of -phenylenediamine (OPD) to generate yellow fluorescent oligomers, accompanied by the fluorescence attenuation of luminol.

The Intervention in Small primary care Practices to Implement Reduction in unhealthy alcohol use (INSPIRE) study is part of a nationwide campaign to improve the identification and treatment of patients engaging in unhealthy alcohol use.
