We report a case of plate-guided distraction osteogenesis to reconstruct a large mandibular defect caused by recurrence of an ameloblastoma in a 17-year-old male patient who had previously had reconstruction using a fibula bone graft.

Distraction osteogenesis and bone transport has been used to reconstruct bone loss defect by allowing new bone to form in the gap. Plate-guided bone transport has been successfully described in literature to treat bone loss defect in the femur, tibia, and mandible. This study reports two cases of fracture of femur with segmental bone loss treated with locking plate fixation and bone transport with Ilizarov ring fixator.

This report describes mandibular reconstruction using a 2-step bone transport technique in a cancer patient with type 2 diabetes mellitus (DM) who received radiotherapy before ablative surgery. The bone, measuring 90 mm in length with the mandibular curvature and good alveolar form, was regenerated using a plate-guided method. Three implants were placed 21 months after completion of the first distraction and 14 months after the second distraction.

Background: Transport distraction osteogenesis has been used to reconstruct continuity defects by regenerating bone and soft tissues. A challenge has been to maintain the correct vector during the distraction process. A new type of distraction device was recently developed that uses a standard reconstruction plate to "guide" the transported segment of the bone.
