[Perioperative diagnostic pneumoperitoneum for unilateral inguinal hernias in children].

Published: February 1991

Fifty-six pediatric patients (2.5 months-13 years old) presenting with unilateral inguinal hernia underwent intraoperative diagnostic pneumoperitoneum (Goldstein test) to evaluate the controlateral groin. Fourteen patients (25%) had positive results and underwent controlateral hernia repair. The positivity is found more frequent in females (37.5%) and in the left sided hernias (50%). The percentage of positive tests has found to be inversely proportional to the age of the patient which is 33% for children under 6 months of age and 18% over 24 months. It is also observed that none of the children with negative test admitted with the complaint of hernia within two years following the operation. Intraoperative diagnostic pneumoperitoneum is a rapid, safe and accurate means to evaluate the contralateral groin in children presenting with a unilateral inguinal hernia. The test eliminates the high negative exploration rate and possible injury to cord structures with routine bilateral exploration.

Laparoscopic cholecystectomy has become the gold standard for treating symptomatic cholelithiasis due to its minimally invasive nature and faster recovery times compared to traditional open surgery, but it is not without risks. A key component of this procedure is the creation of pneumoperitoneum. This is achieved by insufflating the abdomen with carbon dioxide (CO2).

Background: Transperitoneal approach to robot-assisted pyeloplasty (RAP) have been preferred in the last decades because of the use of multi-port robotic platforms. However, this approach is linked to notable issues, such as pneumoperitoneum and lateral decubitus position, which is associated with potential soft tissues injuries, and it is a time-consuming procedure. Single-port (SP) platform was introduced to potentially address these issues.

An elderly patient with renal cell carcinoma underwent a robotic nephrectomy. After an uneventful intraoperative period, soon after extubation she developed generalized seizures and was diagnosed with posterior reversible encephalopathy syndrome (PRES) on neuroimaging. Management included antiepileptic and antihypertensive therapies, necessitating intensive care and neurorehabilitation.

Emphysematous pyelonephritis is a necrotizing infection of the renal parenchyma by gas-forming organisms, with a risk of the gas extending into peri-nephric or para-renal spaces and in advanced cases, the involvement may be extensive and bilateral. It is a common complication in patients with long-term diabetes, primarily caused by Gram-negative organisms or, in some cases, anaerobes. The diagnosis of emphysematous pyelonephritis is made by clinical features and confirmed by computed tomography.

Meckel's diverticulum, a congenital anomaly of the omphalomesenteric duct, is a rare cause of acute abdomen and poses significant diagnostic challenges due to its variable presentation and complications such as perforation. We present the case of a 19-year-old male with a 24-hour history of right iliac fossa pain progressing to generalized abdominal tenderness. Laboratory results showed leukocytosis and elevated C-reactive protein, while CT imaging suggested pneumoperitoneum and an inflamed Meckel diverticulum.
