Background/aims: Infants with diabetes insipidus (DI), especially those with impaired thirst mechanism or hypothalamic hyperphagia, are prone to severe sodium fluctuations, often requiring hospitalization. We aimed to avoid dangerous fluctuations in serum sodium and improve parental independence.
Methods: A 16-month old girl with central DI, absent thirst mechanism and hyperphagia following surgery for hypothalamic astrocytoma had erratic absorption of oral DDAVP during chemotherapy cycles. She required prolonged hospitalizations for hypernatremia and hyponatremic seizure. Intensive monitoring of fluid balance, weight and clinical assessment of hydration were not helpful in predicting serum sodium. Discharge home was deemed unsafe. Oral DDAVP was switched to subcutaneous (twice-daily injections, starting with 0.01mcg/dose, increasing to 0.024mcg/dose). The parents adjusted daily fluid allocation by sliding-scale, according to the blood sodium level (measured by handheld i-STAT analyser, Abbott). We adjusted the DDAVP dose if fluid allocation differed from maintenance requirements for 3 consecutive days.
Results: After 2.5 months, sodium was better controlled, with 84% of levels within reference range (135-145 mmol/L) vs. only 51% on the old regimen (p = 0.0001). The sodium ranged from 132-154 mmol/L, compared to 120-156 on the old regimen. She was discharged home.
Conclusion: This practical regimen improved sodium control, parental independence, and allowed discharge home.

Ethnopharmacological Relevance: Danggui Buxue Decoction (DBD) is a classic traditional Chinese herbal formulation, composed of Astragali Radix (AR) and Angelica Sinensis Radix (ASR) in a ratio of 5:1. It is a traditional Chinese medicine classic prescription for nourishing Qi and Yin (vital energy and body fluids), and it is effective in treating various clinical diseases. Diabetic nephropathy (DN) is categorized under "thirsting," "edema," and "turbid urine" in Traditional Chinese Medicine (TCM).

With the popularity of smartphones, their impact on the physical and mental health of left-behind adolescents has become the focus of social attention. Some believe that smartphone use will provide emotional compensation for left-behind adolescents, while others believe that the lack of supervision of smartphone use among left-behind adolescents will exacerbate their psychological problems. To explore the mechanisms of the dual effects, we conducted a field survey of 848 left-behind adolescents (male = 446, age between 12 and 19) in Shicheng County in Gannan Region of China.

Poultry production is confronting real challenges, including a lofty projected high demand for animal proteins to feed the future, and the need to adapt to planetary boundaries (global warming) with limited natural resources (land, energy, water). Among the most challenging stressors to poultry production sustainability are heat stress (HS) and water uncertainty, that need extensive fundamental and applied research to identify effective strategies. In that regard, our group has recently developed a high-water-efficient broiler (meat-type) chicken line using water conversion ratio (WCR) as a phenotypic trait and defined the hypothalamic molecular mechanisms controlling drinking water under heat stress conditions.

This study aims to investigate the effect and potential mechanism of Polygonati Rhizoma aqueous extract in protecting mice from gastric mucosal injury(stomach Yin deficiency). ICR mice were administrated with the mixture of pungent substance extract and alcohol by gavage once a day for 6 weeks to establish the mouse model of gastric mucosal injury with gastric Yin deficiency. The modeled mice were randomized into three groups of model and Polygonati Rhizoma aqueous extract administrated at 0.
