Pneumopericardium is a rare complication of chest trauma, mechanical ventilation and cavitating pneumonia. We report a case of a 7-year-old patient with chronic myeloid leukemia who developed massive pneumopericardium immediately after being electively intubated for a diagnostic radiological procedure in the setting of ongoing Enterococcal pneumonia. As intensive care medicine becomes more prevalent in hospitals, we believe that clinicians need to be aware of this uncommon but potentially fatal condition.

Radiofrequency ablation (RFA) is a minimally invasive cardiac catheterization procedure employed in patients whose atrial fibrillation (AF) is not well-controlled on medical therapy. While serious complications after the RFA are uncommon, we present the unique case of a 71-year-old male who suffered from acute respiratory distress syndrome (ARDS) and pneumomediastinum post-procedure. While treatment is typically conservative and not required in most cases, massive subcutaneous emphysema may cause significant morbidity and sometimes life-threatening complications such as tension pneumomediastinum and respiratory compromise. Notably, no instances of self-inflicted air insufflation into the abdominal cavity have been reported in the literature.
