Objective: To evaluate effectiveness of an "Continuum of Care Program" (CCCP) for persons with serious mental health conditions in reducing inpatient use, and building a continuum of integrated care that enhanced employment and residential stability. The program combined components of Assertive Community Treatment with a comprehensive wrap-around program.

Methods: A cohort of 1154 individuals admitted to four outpatient CCCPs between December 2003 and May 31 2004 was identified and followed for 1 year. Outcome measures included clinical functioning level, drug/alcohol use, employment, residential arrangement and inpatient use. Regression was employed to explain changes in outcomes between baseline and follow-up as a function of services.

Results: Statistically significant changes were seen over a 1-year period in all outcomes. Housing, employment and mental health improved, whereas inpatient utilization and level of care need increased. Older individuals receiving higher levels of care at baseline and those with higher case management and medical service utilization reported higher inpatient use. Outcomes also varied by provider suggesting the contribution of workforce differences to outcomes.

Conclusions: Although significant, changes in outcomes were small. Outcome effectiveness was mixed and generally unrelated to services. These findings imply that significant changes in outcomes may require several years to obtain.

Background And Objective: The impact of lifetime body mass index (BMI) trajectories on adult lung function abnormalities has not been investigated previously. We investigated associations of BMI trajectories from childhood to mid-adulthood with lung function deficits and COPD in mid-adulthood.

Methods: Five BMI trajectories (n = 4194) from age 5 to 43 were identified in the Tasmanian Longitudinal Health Study.

Background: Many people with kidney failure start and remain on in-centre haemodialysis treatment despite evidence of improved outcomes with home dialysis. To make an informed modality decision patients must receive frequent, high-quality modality education. This education is inconsistent in the in-centre haemodialysis setting, where patients spend the most time with nurses while receiving haemodialysis treatments.

Background: A 12-month cluster randomized controlled trial (RCT) demonstrated the effectiveness of an application-based education program in reducing the salt intake and systolic blood pressure (SBP) of schoolchildren's adult family members. This study aimed to assess whether the effect at 12 months persisted at 24 months.

Methods: Fifty-four schools were randomly assigned to either the intervention or control group.

Background: Spacer-type tibial osteotomy have been proven a novel and effective osteotomy to treat osteoarthritis, while lack of comparison with other surgical methods in younger patients. This study aims to evaluate the short-term clinical outcomes of spacer-type tibial osteotomy versus open wedge high tibial osteotomy (OWHTO) and unicompartmental knee arthroplasty (UKA) for Kellgren-Lawrence (K-L) grade 3-4 osteoarthritis (OA) in patients younger than 65 years.

Methods: This retrospective study analyzed a total of 224 patients with K-L grade 3-4 knee OA treated from March 2018 to November 2020.
