Importance: More than 4 million Medicare beneficiaries have enrolled in dual-eligible Special Needs Plans (D-SNPs), and coordination-only D-SNPs are common. Little is known about the impact of coordination-only D-SNPs on Medicaid-covered services and spending, including long-term services and supports, which are financed primarily by Medicaid.

Introduction: Family caregivers of people living with serious mental illness such as bipolar disorder, psychosis and schizophrenia, are continuously burdened with caregiving, following the complexities of navigating the mental health system for their loved ones. The aim of the study was to understand the perspectives of caregivers about care coordination for people living with serious mental illness, highlighting the current landscape and new directions across Canada.

Methods: In this co-designed participatory qualitative research, caregivers of people living with serious mental illness, and service providers were engaged and purposively sampled across Canada.

Despite declines in teen pregnancy, pregnant teens remain at risk for poor health and educational outcomes due to a lack of care coordination and access to resources. The purpose of this project was to develop and launch a care coordination toolkit to provide resources to pregnant and parenting youth (PPY) at a large urban school district. School personnel were trained on the use of the toolkit and completed pre- and posttraining surveys to evaluate changes in knowledge and confidence.
